In the US it is possible to earn an “equivalent high school degree” without attending high school. If you are in SMU/SMA then you are getting a diploma, therefore this question doesn’t matter to you.

Correct.

Yes. Same process: transcribe, then notarize, etc.

In the US most high schools use the semester system. Midyear report is released in December or January, which means it is the latest report if you apply in February. I think in Indonesia you use a trimester system, so you should include the latest report, which I think is from October or November?
